Intake Form

If you are a new client and have not yet filled out James’ Intake Form, please do so now. You can click on these links to open them in a new browser, or right-click the link and choose “save as” to save it to your computer.

The form typically takes about 4 hours to fill out and must be turned in to James at least 24 hours before your first appointment. Typically we wait to schedule your first appointment until after you get this form back to us.
